E&M Coding Tip

In performing an Evaluation and Management (E/M) service the urologist
may review and use (without re-performing or re-documenting [if
already documented by ancillary personnel]) the Review of Systems,
ROS, the Past Medical and Family history, PFSH, chief complaint,
CC, and the Vital Signs. He may just review these, sign the note
or make a comment as evidence of agreement and still receive credit
for these elements in determining the level of service performed.
This is certainly a time saver for the physician and insures complete
documentation when needed.
